Same here, anything over 200cm and it goes BAD. Small things come out perfect.
The above has been tried with several default Prusa XL profiles and all have issues with the last layers.
The first layer could not be any more perfect so don't try to pull off the princess and the pea thing.
I only changed top layers to 5, perimeters to 5.
The cooling was default 30%-50%, The temp was default at 240 after first layer.

Strange thing for me is that it's been working great for months and all of a sudden just the top layer seems to have problems.

It looks like warping, if you turn it over you will probably see adhesion problems - on an XL that suggests either a print-sheet in need of degreasing or you are printing in a draft.
Clean the sheet and try adding a brim.

Bottom of the object is fine and I am already printing with a Brim. I cleaned the surface, but no change.
It takes a while to print the object. I'll clean it again and recalibrate over the weekend and try to print it again.
What filament is this? Another cause that we don't see so often nowadays is out-of-spec filament - measure the diameter in a few places and average; if it's off enter the true diameter in Fialement Settings.

I'm using BambuLabs Matte PLA. It's the same filament that's been working fine.
I did increase the temperature to stop warping. I'll mess around with that some more.
