Print gives error because of filament accumulating on top of eachother

Print gives error because of filament accumulating on top of eachother, but I have no idea why it's doing this.
Sometimes it's doing this right away when printing, sometimes it's doing this later on. 
I've tried to print things that it already printed successfully, but these also go into error.

Do I need to replace the nozzle or something else?

Often this is caused by an improperly fitted nozzle. 

Have you changed or cleaned the nozzle recently?


If there is a gap between the top of the nozzle and the bottom of the heatbreak, 
Filament can leak out and typically make a mess on the top of the heatblock, where it will turn black and eventually fall onto your print

I don't have exact images of the prusa setup, but this should help. 

One classic error is using a Mk8 Nozzle, on a Prusa Hot end. 
the screw thread on the Mk8 nozzle is about one millimeter too short!

Alright thank you for the great pictures, I will try to fix this.
We have replaced the hot end, maybe we put the nozzle back in the wrong place.

As Joan said, this is often an indicator of a leaking hotend. Filament manages to get through a slot between nozzle and heatbreak, runs down the heater block and drips on your model. Less severe cases could be caused by filament build-up on the heater block or nozzle, perhaps from a blob of doom and or insufficient care in cleaning the nozzle and heater block between prints.

And one more perhaps esoteric cause—but one I just encountered yesterday. If you slice a model for PLA but then load PETG (yeah, stupid me), the first layer goes down usually okay but then all hell will break loose on the next layers.

Thanks for the info guys, we managed to fix the problem. We screwed in the nozzle a bit further and that fixed the problem!
