Z Axis Calibration Issues

We have a Prusa Mini which is acting very odd.  Whenever we try to do the mesh bed leveling or calibration it keeps having issues with the Z axis.  The head keeps moving little by little away from the bed during Mesh Bed leveling and the calibration wizard sends the Z axis to the top but it never comes back down and either gives an error or sets the z axis 0 point about half way between the top of the bed and the top of the z axis. 

Needless to say, I can't print anything on this printer.  It's almost like it loses it understand of where it is on the z axis so it doesn't know how far up or down the head has traveled.  Minda is powered and shows light on top and we have unplugged the z axis motor and plugged it back in.  We have been staying up to date with firmware so it's running the latest 4.1.0 code but it's still doing this.

The Minda is broken. Contact support and they'll send you a new one.
