Y axis belt keeps slipping out of tensioner!

I bought a pre assembled MK4 back in January. All of a sudden the other day the Y axis belt slipped out of the tensioner. When I went to fix it I noticed that they forgot to put the screws in that are supposed to hold the belt in. I put some in and reassembled it and it worked fine for a print and then failed again. It's done this a third time now and I'm not sure what to do to fix it so if doesn't keep happening?! 

I'm sure it doesn't help that I'm printing ASA mostly, in a Lack enclosure. 

I'm about ready to glue them in, but I don't really want to because that'll make it extremely difficult to repair or anything later on.

Does anyone have any suggestions?

I think it's the belt ends slipping out of the holders, so I'm not sure that loctite would really help. The screws that are supposed to pinch the belt in are still in place.

probably silly of me to propose this, but since you bought it preassembled, isn't this a warranty case and should be addressed by prusa support?

Good point. I did talk to them the first time but I thought I had it fixed. I'll get ahold of them again today.

Their suggestion is to reprint it in ASA of course. I'll give it a try and hope that took hold together long enough.
