Underextrusion only when using PrusaSlicer

I recently purchased a Tenlog D3 Pro (Dual Extruder System) and it comes with a Cura profile. Sloppy but it gets the job done. However, I much prefer PrusaSlicer so I transferred over all the settings from Cura to Prusa and boom, most of my issues disappeared. Unfortunately, now it seems the first extruder has pretty decent underextrusion despite having as many of the same settings as Cura as possible. The underextrusion only occurs when it starts printing, which if I am running only one extruder is fine, a skirt takes care of that. The problem is when I do an Extruder Change back to the first extruder, it starts and underextrudes for a while before it actually gets started. Everyone just says to add a Wipe Tower, which is fine, but Cura can do it without a wipe tower. Cura has no underextrusion whatsoever. Not sure if this is relevant but Prusa prints underextrude in general, but struggles when trying to move to a bunch of different spots for small details.

I have dug into the Gcode (novice) and I can't see any major differences that would cause this issue.

I wasn't sure what information to include, but please ask for anything you might need. I included the txt gcode files for Prusa and Cura.

Again to clarify, the second extruder has no problem whatsoever. Using Cura to print, neither extruder has any problems. The settings are all the same where I can manage it.
