Trying to punch a hole into the side of a hollow object.
I have a cylinder, with zero percent infill. If I put a modifier on the top or bottom, with zero perimeters and layers, the hole works as expected. If I do the same to the side of the cylinder, the modifier with the zero perimeters and layers, just creates an indentation, not a hole. If infill is enabled at 5%, which is not what I want, the hole works correctly. How do I create the hole in the side of a hollow object?

A cylinder, with zero percent infill is not a hollow object - it's a solid object that happens to be mostly void. If you cut a negative shape from it that shape will cause new exterior to be created...
Make a hollow cylinder by making a full sized cylinder then subtracting a narrower one from it concentrically. Now proceed as before and it will behave as you expect.
