What could be causing this?

Any ideas what could be causing this?  

Printer is only a month old, first few prints no issues, then this sort of thing started to happen.  Doesn't happen all the time either.  I re-calibrate both heads pick-up, lubed the rails.  Every time it does happen, its generally at the same elevation.

Try the belts and the Homing Tower at first

Probably part warping causing a crash.
