To be honest, I'm not 100% sure.

But, changing first layer extrusion width helped a lot. I still can't use flow 100, it is just not extruding enough, but I'm down to 120.

I also removed these three lines from the start gcode:

M221 S{if layer_height<0.075}100{else}95{endif}
{if print_settings_id=~/.*(DETAIL @MK3|QUALITY @MK3).*/}M907 E430 ; set extruder motor current{endif}
{if print_settings_id=~/.*(SPEED @MK3|DRAFT @MK3).*/}M907 E538 ; set extruder motor current{endif}

Bontech also advised me to raise the current with: M907 E550.

I set first layer speed to 30 mm.

I raised bed temp to 70 degrees C.

Also I used https://forum.prusa3d.com/forum/original-prusa-i3-mk3s-mk3-assembly-and-first-prints-troubleshooting/life-adjust-z-my-way/ to calibrate Z.
