Random dramatic x-axis shifting - help!

Dear all,
I am very new to 3d printing and built my first (the i3 Mk3) only 3 weeks ago, so I would dearly appreciate any help/advice on how to fix my x-axis.

Here is the problem: I get these seemingly random layer shifts (see pic).

Obviously the x-axis is not behaving well but my belt seems straight and taut. Before I start pulling the x-assembly apart, does anyone have any advice?

Check the set screws on the X axis pulley that lock it to the stepper motor shaft.

If these screws are not tight enough the motor shaft can slip and not accurately drive the pulley. The screws can work themselves loose over time, especially if they weren't quite all the way tight to begin with.

Now I'll be sure to make sure those nuts are tightened.
