Extruder failing during 1st Layer Calibration

So anyone have any idea why this is happening? I've fooled around with tension and temperature and z-offset and you name it till I'm blue in the face, but it never fails to fail. During 1st layer calibration it does the zig-zag just fine, but about halfway through the rectangular block it prints at the end of the test, the extruder starts to extrude inconsistently until it pretty well gives up.

If I go ahead and start a print, the same thing happens... the extruder extrudes for a bit and then it fades out and dies altogether.

Removed and reinserted the filament? 

Is nozzle clean? 

Is nozzle temp stable? 

Bondtech wheels, are they aligned? Are they not clogged from the plastic? Is the door to bondtech wheels firm but not too firm?
