What can have caused this failed print?

What can have caused my >25h print to fail in the last half hour? It was still printing when I pressed the panic button.

(MK3 S+, PLA, printer 3 months old, less than 100h total printing time. Failure started at 189mm height)

If you look closely it seems like the printer suddenly moved 60mm right and printed about 20 layers there. Then it moved back to correct position and continued printing.

Two days ago a similar model failed, also with less than 10mm height still to go (PET-G). But I didnt look at that one as closely, so I don't know if it showed the same right - print - left print error.

Before that I have only printed smaller models, with good results.

There is a good document on layer shifts.

https://help.prusa3d.com/en/article/layer-shifting_2020

Have a read through, see if this points to anything

Tank you very much!

Thanks,

I did read that before posting, but not thoroughly enough. Missed the belt tension cause.
I ran a belt test check and my x-axis showed 237-240. So I should loosen it according to the recommendations. I'll try that and pray my next big print succeeds.

But how come the printer managed to get back to the right position?

If after the layers shift the extruder crashed into that shifted part that could have triggered a crash detection. And that will reset the printer to the original place after homing x and y.

Sounds like it might be the issue.

Will keep my fingers crossed for you, nothing worse than failure after that long, it is soul destroying.

It wasn't the x-belt tension. The motor bracket is colliding with the spool holder! When I was cleaning and lubricating the x-axis I noticed a sudden increase of resistance when moving past the middle of the printer. It took me while to realize why, but then I saw that the motor bracket is barely touching against the spool holder. See the pics.

Has anyone else seen this issue? I tried searching the forum/web but couldn't find anything.
Is my assembly wrong or was I just unlucky with the tolerances?

I think I will sand down the spool holders lower grip to get clearance.
