Likely cause of slow oozing around nozzle?

I am getting a little bit of filament oozing out around the nozzle at the point where the nozzle threads into the silver heat block.  It's not much, but after a 6hr print there's enough there that gives me cause for concern.  At rare times a little bit of that has come off onto the current print.  

What is the likely cause of this?

PTFE tubing not fully inserted or heatbreak not correctly inserted.

Thanks.  I think it likely that the heatbreak has untwisted from the nozzle a little.
