Endless Under-Extrusion, Please Help

I'm at my wits end, I've tried the below already for troubleshooting:

Heatsink on extruder servo motor - Trying to prevent heat creep
Cold Pull - see embedded photo, thats the last cold pull I've got
Increased temp - raised the PLA temp to 222C in an attempt to reduce strain on extruder motor
Reduced the retraction setting - reduced retraction length from .8 to .5mm
Re-sliced model - in case the file was corrupted I've tried several times to re-slice the stl's to eliminate this possibility

Cold Pull

The good news: At least it seems to fail in a repeatable way at about the same point in the print.

A few things to check:  Have you checked the spring tension on the extruder gears?  Also, are the gears properly aligned with the filament path?  No filament knots in the spool?

@dimprov

no knots that I can see, the tension spring screw is a little farther out than flush with the nut.
