What causes this (misaligned layers)?

What causes this (misaligned layers)?

I printed the exact same G-code 3 times. See the attached image for the three different results. The misalignment is in the Y-axis.

The prints were done in PLA with default Cura Normal Quality, however I've experienced the same phenomenon randomly with the Prusa Slic3r on other files.

A few things:

The pulley is too close to the motor
The pulley grub screw is loose
The belt is loose (slipping)
Printing in "quiet" mode

Plus a few other less likely things.

My guess would be a very loose belt, or the motor gear loose.

Tightened the y-axis belt and haven't seen it since. Thanks!!!
