Print Failed and I found this Set Screw in the print

Been printing in ABS. Printed this exact part twice before and was just printing a 3rd copy. Print was about 60% complete and then failed.

As I was cleaning it up and trying to figure out what happened I found this set screw in the print. I assume it fell out of the printer.

1. Where did it come from?
2. Can I fix this myself?
3. Why did this happen?
4. What else should I check before I resume printing?

This is probably the setscrew for the x-axis pulley. Just screw it back in there and maybe add a drop of screw-glue to it.

or the Thermistor retaining screw for the hot end!
