Heater can't get hot enough

My printer has been out of action since the end of February. First I got a clog. Then in clearing it my hotend cooling fan threw a blade. So then I ordered a new fan, and installed it. Now my heater can't get it (the temperature) up anymore. I set it to do a preheat to 215 degrees. It climbs up past 50, fan comes on. Then it climbs up to around 115, and starts to slow considerably. Then it gains around one degree every 30 seconds. Then it just plateaus at around 150 degrees.

Is my new fan blowing too hard, or does that sound like my heater block is fried? Burned out heating element? Could it be that plastic is clogged inside the block? After it gets to 150 degrees I don't see any signs of smoke, so I'm not sure there is plastic melted in there. I just don't know what to do next in the troubleshooting...
