Retractions in g-code not always honored? / What's going on here?

The gcode looks fine. Most of the layers actually print fine. But on some (seemingly randomly selected) layers, it seems like the extruder continues to push material while traversing to a new perimeter start location, causing erroneous bridges. Here's a photo:

As an aside, this print failed because the printer appeared to lose its position somehow. It started trying to print offset in X by about +10 mm and in Z by about -5 mm. Weird.

Are you printing with stock Slic3r settings or have you modified things?

Printer settings (and thus retractions) are stock.

Filament and print settings are not stock. Is there a particular setting that seems likely to cause these random non-retractions?

Where are the non-retractions? Are they hiding under the lines left when the filament going round the corner did not adhere to the layer below and were left exposed when they contracted as they cooled?

Interesting thought; I hadn't considered that it might be a non-adherence to the previous layer. I have other examples where that can't be the case, however - on convex outer perimeters, little loops that couldn't possibly form due to material getting pulled off. I'll post a picture next time I print an object exhibiting that.
