Part Cooling Fan Never Turns On During Print

I am trying to print (in PLA) some small square post about 30mm high, 5mm wide, and about 5-6mm into the print, the post gets deformed. I guess the best way to describe it is that the square just kinda deforms and takes on more of a circular/globby shape. I noticed that the cooling fan is not turning on at all. I believe this is causing the globbieness because the temperature is so high due to no cooling on such a small part and the layer times are so short. I performed a fan test in the calibration menu and the fan turns on and passes. I've tried using different PLA presets in PrusaSlicer as well as manually just setting it to always on, even on the first layer, and still it won't turn on.

Is there something possibly wrong with my fan or am I just overlooking something simple? I've reinstalled everything as well as updated to the latest firmware. The only thing I haven't done yet that I can think of is double check the wiring on the board, but I don't really think that would be the problem since it passes the test fine. Any help is appreciated!

In Prusa Slicer, look at Filament Settings > Cooling tab: make sure that Keep fan always on and Enable auto cooling are turned on.

They are definitely on. I have checked the gcode as was recommended in one of the google results I found and the M106 command was there and set to 255 as it should have. I've also tried disabling auto cooling and forcing the fan on always, even on the first layer, just for testing purposes and still no luck.
