Given the length of the hex flats, this looks like the "E3D Prusa Nozzle: High Flow Brass", not the "Prusa Nozzle brass CHT". I wonder if the tip insert fell out for some reason. Too much pressure in the nozzle from aggressive cleaning attempts?

If this is a nozzle purchased through Prusa or an official representative, get in contact with them. They most probably will replace it.
