Filament missing entry point to nozzle

I am finding that the filament would not load into the nozzle tube for some reason on my MK4.

After it passes the sensor, it seems to get stuck in the gearbox where I think it is meant to be guided into the tube that leads to the nozzle. Instead, it misses the hole.

The loading process is loud with a clicking sound (switching between around 8 Hz to maybe 20 Hz) and I suspect that noise is the gear pushing the filament away.

When I open the idler to inspect, I can see how the filament (Prusa PLA) is deformed. It basically has no chance of entering the tube.

I was on a chat with Prusa and I was asked to look at the idler nut. I was told they looked ok from the photos I sent. As can be seen in the photos, the filament gets bent and then, when I pull it, it breaks off.

Anybody else seen this? Any ideas for what to do?
