Turning Sprial Vase Mode off doesn't stop slicer from using it.
I've noticed that it takes a few restarts to turn spiral vase mode off in the slicer. I uncheck the box and PrusaSlicer continues to use it.

After you have turned it off you still only have 1 perimeter selected and 0% infill. So how else do you expect it to slice ?
If you were to print both files you should see a slight difference as with vase mode there wouldn't be a retraction on each layer and with it off there would be (assuming your extruder settings have retract on layer change of course)
Also try turning on retraction/detraction visibility on the preview and scroll through the layers. Then compare the previews of the 2.

Ah I see. I guess flipping vase mode on turns infill to zero? I was just flicking it on and off without adjusting my settings. I guess I should have two separate profiles.

Yes it changes several settings potentially. As well as infill there's top layers, perimeter count. Turning it off does not restore the settings to what they were before.
And yeah its normal to save a specific profile or profiles for Vase mode. When you print vase mode it is also common for users to change other things such as the perimeter extrusion width for example. Even with a 0.4mm nozzle, setting it to 0.6 or even 0.8 isn't uncommon. It can lead to much better vase mode prints.
