Slicing problem with g code

I am new to 3d printing but have successfully printed some test prints on my new mk4.  The firmware is also up to date.  I test printed one of my grip designs, it came out too large, but was dimensionally accurate, so I scaled it down to 80% in Fusion 360.  I saved it and ran it through Prusa slicer, the preview looked good.  On printing the resized model, every 5mm in height the layers shifted forward 10mm on the z axis, I aborted the print.  I printed something else from printables to check the hardware and it printed fine with no issues.  I re-sliced the model under different parameters and everything looked good in preview, the same problem occured when printing.  I tried binary g code, the layer shift changed to 5mm jump from 10mm.  I don't understand why this is happening.  I attach photos for reference (top one is as designed, lower one shows the problem.  Any suggestions would be much appreciated.

This looks like a mechanical problem rather than a slicing or gcode problem. Something is slipping or getting stuck and causing lost steps in the X or Y stepper motor. Check that the pulley on the motor shaft is well secured and the grub screw is tight onto the flat of the shaft. Check that the belts are not slipping or too loose. Check that the axes move freely when the machine is turned off.
