Exporting PrusaSlicer config.ini doesn't always have the changed settings

On several occasions I've noticed that after modifying settings the exported config.ini doesn't contain these changes until after I save the modified settings to new user profiles. Kind of annoying since I want to be able to change stuff, export a config and do a comparison without having to save these modifications first.

Why would you expect it to ?  Changes are ephemeral until they are saved 'somewhere'.  Either in a profile or as part of a project file.  The Config.ini is where they are saved to in the normal course of things, so of course the export doesn't contain any changes until they are saved.

I get that. But wouldn't being able to dump a config without saving be useful? Especially if I want to compare changes using diff.

Possibly, but you can do that inside Prusa Slicer using the compare preset tool  at the moment.  Your ProfileXXXxx (Modified) is available in the dropdown to be compared with any of the other presets.

If you want a diff using an external bit of software then just save your changes to make them an actual file.  You are just editing stuff in memory until that point.
