Nozzle scratched my plate

How can I make sure that my nozzle doesn't scratch the plate?

I started a new print the other day, it did the bed leveling and then proceeded to start the print as expected. However, on the first pass it made a large gouge in the plate and then the printer alerted that the filament was stuck which stopped the print. Note: The filament came out just fine on the purge right before it started printing.

I thought that bed leveling would have prevented the nozzle from going too low on the plate.

I'm running firmware 6.4.0 on a XL 5 head printer. I haven't had this issue until now. I only recently updated to this version of the firmware. I haven't used this particular head since that update but I don't know if it is related. I used a level to check the balance and it does look slightly off. Would that have caused it?
