Extruder Filament Sensor Calibration

I am getting inconsistent loading due to the filament sensor. I tried the official calibration and I still see the sensor reporting no filament when I move the filament in and out some without removing it. I.e. I insert a piece of filament/ and move it up and down in the extruder and can see the sensor switch between 1 and 0. 

I am printing the below to make it easier, but wanting to see whatever other ideas everyone has. Everything on the printer is working good except the reliability of the filament sensor.

I have checked my screw tightness on the idler and it moves freely.  Not sure if I need a different idle door also like this:

Bad design (in my opinion) by Prusa for this. Tuning the IR was always my biggest PITA with the MMU until I switched to my Bondtech extruder which uses this mod for the MMU. It's really nice as it puts the IR board in the shuttle on top and you can adjust it's position independently of the idler door.

I see they have done a version for the stock MK3S extruder, but I have no idea (and the details say he doesn't either) how well it works,
