Extrusion problem

I've been happily printing for the past few months. Lately, I've hit a problem, which I think is caused by not enough filament getting extruded. The end result is separated threads on the resulting print.

Bringing the temperature up (PLA, from 210C to 240C) and increasing the flow to 130% helps a bit, but not too much.

I changed the nozzle with a brand new one, but the problem persists. I noticed that when changing filaments, there is an edge in the retracted filament, so I suspect there is some clog in the extruder body. Any tips on how to solve this?

Maybe the PTFE tube wore down. You can check this guide we made. http://manual.prusa3d.com/Guide/How+to+replace+PTFE+tube/82

Instead of completely replacing the tube you can just push it a little bit deeper and cut the degraded end.
