Slicer Print Settings -> Infill -> "Top/bottom fill pattern: " Should be rectilinear or concentric for a smooth top. Yours looks like "Hilbert"?

i am using the Prusa original settings, the infill pattern is already in Rectilinear.

Really hard to decipher the photos.
Can you show a Slic3r photo that is not all supports, but zoomed in on the top surface?

in your top picture, there are red lines, the part that is poor finish?
it looks to be a gentle slope, which is likely to render as a series of steps, each, equivalent to the layer height. Gentle slopes are prone to displaying these contour lines. the top slope may exhibit similar but less severe marks the slope is more steep so the steps will be closer together.
if this is the problem, then variably layer height may help, if you make the layers that make the slope, as thin as possible.
from what I can see of the model, the top overhang should print without supports, unless there is more severe overhang hidden from view...

Thanks for your information. You are absolutely right. The surface of the base has a slope and the series of steps depends on the layer height. Here is the photo of the slicing with 0.05 layer height. Also it might help if I print with a different orientation, that would get rid of the contour lines. thanks everyone for the help.

I have the same issue and turns out it was something to do with the STL file. Sic3r for what reason does not slice the last top layers, leaving the infill exposed. I did try fixing the STL file but I was not successful and ended up having to use Cura
