Slic3r Part Cooling Fan settings don't seem to work consistently

When I'm printing a small object with PLA, say a 20mm cube or a Benchy for instance, I set the part cooling fan in Slic3r to "Keep Fan Always On" and set the fan speed to 100%. But it never keeps the fan always on. It will kick on from time to time, but not enough to cool the part as it is printing, causing defects in the print, such as curling. Also, when not printing, I can manually turn the fan on, and the fan works just fine.

Has anyone else run into this? I'm using the Prusa Edition of Slic3r (v1.37.1): http://d.pr/i/m9gBM0

Would you please create a Slic3r issue and provide the STL and config.ini? (you will export the config.ini by the menu file->export config).

I believe it is a configuration issue, but I want to make sure we don't overlook some bug.
