Under Extrusion at Start of Some Middle Layers

I have not see this before.  MK3S+, PrusaSlicer 2.6.0 beta3, Jessie PET filament, printing just one item.  There is under extrusion at the start of some of the layers, but only the layers with the pockets on the opposite side of the print.  The end point for the previous layer is relative close to the start for most of these layers, and is further away for some of the layers outside this zone that start properly.  I have printed at 0.2mm layer height as well as variable with the same results.  Standard filament settings, and print with avoid crossing perimeters enabled.  I swapped to a 0.6mm nozzle (I use Revo) and the results are worse.

Any ideas what might be causing this and how to fix it?

Looking closer, it looks like detractions occur at that point.  I have never had to play around with those settings before.
