How do I assemble a kitted hot end?

I was forced to buy a kitted hot end due to no assembled hot ends being available. I have searched for help on assembling it but found none.

Any help would be welcome.

Thank you, that was helpful. Though my print head configuration differed slightly this gave me enough direction to get the thing working again. Flimsy thermistor wires were the problem. They snapped when I was trying to de-glob my print head after a failed print.

they are a problem. 
I use a soldering iron to carve the bulk of a blob off, (whilst heating the blob from the heater as normal).  to save stressing the thermistor and heater wires.
