Extruder problem

I've got a severe problem with my i3 MK3S.
The first layer is always printed 100% OK.
When printing the next layer, the perimeter is also printed fine, but after that gives the Extruder some strange clicking sounds and no new filament comes out.
I have tried printing different items, but always with the same result. First layer always OK but after that it can't pull out new filament.
Please help.

Try tweaking your extruder idler pressure.

but I already tried that without any help. I think I will have to give up repairing the machine.
In any case, I have no imagination for what I can do

Whar filament are you using?

Please show us pictures of a failed print.  One on the print sheet and another showing the underside.

I use Prusa PLA

It is a Prusa original i3 MK3S.

I wonder if clicking is the extruder gears being loose and/or out of alignment.  As things heat up just enough to cause things to slip.  Possible clogged teeth on the extruder?  I have seen that they need to be cleaned from time to time.  My printer is new so I have not run into it.

Both gear wheels are completely intact, and they are not loose.
But now the situation has become much worse. Now I can't even install new filament.
So I guess it's all due to the extruder being full of filament which is stuck.

So my next question is how do I get the extruder cleaned?

If the motor would need to use more torque to turn the extruder gear thant it can deliver it will start to make "clicking" sounds. They are quite typical so if you know them, you can identify these motors sounds quite well. There are other possibilities of what could click but in most cases I would assume it is a clogged nozzle. Maybe it is also just partially clogged. 

When you load a filament, does it work? Does filament come out as it should? Do you hear any clicking sounds?

In any case, I'd open the extruder gear chamber and see if it needs some cleaning out or if some filament is mangled in there, then reassemble it and do a cold pull. Possibly repeat it if the pulled out tip doesn't look as intended. There is a cold pull tutorial in the guides section of the Prusa page.

I'm quite sure now. It is a blocked nozzle.
But I have decided to send it to a specialist I know for repair.

But many thanks to everyone who tried to help me.
