Inconsistent first layer following clog. Bed leveling issue or clog in PTFE/nozzle? (Video Attached)

Inconsistent first layer following clog. Bed leveling issue or clog in PTFE/nozzle? (Video Attached)

Had my second clog in one month during a small print where filament completely stopped extruding.  Had to remove it from the gears with pliers.  Followed up by pushing some PLA manually through the heated nozzle.  Didnt notice any unusual debris or color coming from the nozzle.  Flow seemed okay.

Decided to print a first level square (3"x3"?) to examine.  Half of the square seemed good, but the 2nd half had some gaps between the lines and didnt look like a good first layer.  Decided to run the full bed size 3x3 bed level troubleshooting print.  Seemed somewhat inconsistent, with the middle being the worst offender.  What steps would you take from here?  Movie attached.
