Consistent Benchy Failure

Hello,

every time I try to print a benchy, it fails at the same point every time and the print looks bad. It will get kicked off the build plate and there are lots of small shifts in happening. I’ve checked the adhesion (washed plate with dawn and rinsed thoroughly), checked the idler tension bolt to see if the filament is feeding correctly, checked that the bed is flat (via mesh test), checked the belt tension, and made sure the set screws on belt pulley are tightened. Feeling quite defeated. Any ideas?

you can see that even the first couple layers are having trouble in the photo below

Here is the bed test print and failed printer where the extruder head kept running itself into the first couple layers of a print
