Any idea whats wrong with my top layer?

I dont know what information is and isnt relevant so I'll just go through everything.

Printer was running fine, no complaints besides the fact that the thinnest I could get a calibration layer was 0.22mm, not 0.2mm.

I noticed some weird layer shifting in the prints, which I eventually narrowed down to filament getting caught under itself. Honestly think I just tucked it in to store it and forgot to untuck when I next loaded it.

After fixing this I tried printing a model at 0.05mm layer height for the first time. I ended up with some blobs and stringyness, which was likely an issue with z hopping from what I've read. I put that on hold and went back to what I was doing with 0.2mm layers.

And then this top layer scarring started happening...

I thought maybe the caught filament messed up z calibration, so I re did that, no dice. I then recalibrated offset. First attempt was a clean 0.2mm exactly, second attempt was 0.22mm again. 

I then tried looking into z axis issues. My z axis made more noise then any other, but since stuff has been fine so far I never touched it. I tried loosening screws on the top and bottom, running the axis up and down, and tightening them again to try and re align the axis. I also tried loosening the screws on the z axis nuts because I heard those being too tight were a problem. Noise got a little better, re ran z calibration, annnnnd top layer scarring happened again. I'm at a loss, any ideas?

Are you sure those marks are not from the nozzle?

Save, zip and post your slicer project (3mf), this way all your settings and tweaks are visible to everybody.

What do you mean by that? Did you print one layer and measured it?

1) If you extrusion is not dialed in perfectly, you will get problems like this. Thicker layers are more forgiving in this regard.
2) With a 0.4 nozzle you should stay above 0.1 layer height.
