Thermal Runaway with cooling fan on more than 15%

My printer seems to have a lot of thermal runaway issues lately. It most often happens during bridging when the cooling fan goes on max for the filament profile. I've tried turning the fan down which seemed to work for a couple prints but at this point I can't get the hotend to stay at 260 with the fan on more than 15%. Any ideas what might be wrong?

This is on an MK2 with the latest firmware and gcodes generated by Slic3r.

Hi Colin,
what are you printing that needs 260 c for the filament, and the fan...?

I have used ABS at 255, but didn't need the cooling fan... I don't use ABS at all now, as it gave me chest issues.

can you let us see a picture of the extruder from the front ?
the part cooling fan does tend to blow on the hot end, a bit... a silicone sock would help reduce this effect.
if the extruder is incorrectly assembled, the heat block will get a lot of cooling from the part cooling fan

I had thermal runaway on a small piece that was centered on the print deck by Slic3r. I read that the deck heat is measured there, and can pick up the nozzle heat. I added a large thin circle to my design, displacing my part away from the center and it printed well. Just a thought.
