Loose Pulley Y Axis Selftest Failure

Hi all, I recently ran into a problem where I could not print.  The initial setup said that the bed had leveling issues.  I decided to re-calibrate the printer.  However, I keep getting "Loose Pulley Y" errors during the selftest.  I adjusted all the screws, the pulley, checked the connections, and everything seems fine.  Any ideas what the problem can be?  Video attached.  Thanks in advance!

To me it sounds like a loose belt. That is what I think I hear when the bed moves back to the front after first hitting the end stop.

If the belt skips the printer will see that it has to make more steps to hit the end stop the 2nd time. A loose belt or loose pulley will look the same to the firmware.
