X Axis binding randomly

I got my MINI+ kit a few months ago and put it together with no problems and it has been printing just fine for a while. Lately, though, I've started getting layers randomly shifted along the X axis. Sometimes the entire print will be fine, sometimes it's a single failure, and sometimes it's shifting all over. Here's one of my most recent prints, all of these layers are meant to line up. (click the picture to see the edges where the problem is readily apparent)

I've tried changing the tension on the belt either way by loosening and tightening the bolt on the end but the problem remains. When I run a self test it always fails on the x axis. Autohome also tends to not get to the right place on the x axis. Manually moving the axis via the settings seems to work just fine. If I push the x axis manually it seems to bind up at various places but it's somewhat random.

That looks Nasty, 

does your X axis run freely when moved by hand? 
did you lubricate the linear bearings during assembly?
 
does your X axis idler turn freely?

Does your X Motor pulley rub on the surrounding components?

I actually figured out this problem after disassembling the entire x axis. The set screws on the motor were loose and the gear was slipping. I think that the assembly instructions had told me to leave it not fully tightened and then I didn't see a place where it told me to tighten them an I forgot.

Yes, it happens many times that the cause of failure is because we overlooked something during assembly, or we didn't realize the importance of certain assembly process -especially if issue starts later after successful printing.
