Extruder Gear unable to move/grinds against Filament

My extruder gear is unable to grip the filament properly and, as a result, just grinds against the filament without moving it. I have cleaned the gear multiple times, have adjusted the idler screw's tensions across all ranges, and have checked whether the problem is with the PFTE tube I'm using, all to no avail. For some reason when I press down on the piece of plastic responsible for the tension of the gear extremely hard it works fine, yet does not work even if the idler screw is screwed all the way in. Any suggestions on what to do?

1. Maybe you have to clean the nozzle, so it flows out more easily. ? If a "partial" blob is in there. You may have to get it out. 
2. Correct nozzle size in settings and gcode? When real nozzle size is smaller then the size set in slicing, it pushes more filament then the nozzle can manage. 

We will do what we have always done. We will find hope in the impossible.
