Bed leveling repeats a row and skips the last, sometimes skips the left-most column

Sometimes when I run the bed leveling routine the printer will repeat the 3rd row and then skip the last, it'll also skip the left-most column and the X-carriage will ram into the Z extrusion. If I home the printer beforehand it'll usually perform the bed leveling normally. I've checked the SuperPINDA indicator LED functionality with a steel hex key, jiggled it while moving the x-carriage and did not notice anything weird. The printer passes the XYZ axis test, I've checked belt tension on the X and Y axis with the printable Prusament PETG gauge and it looks within range. It doesn't indicate any error on the screen after it has stopped probing, so I'm not sure what's wrong with it. When it probes correctly I'm able to print without issue.

or you have overtightened the Y-axis belt.
