Z-axis problem causing dent in bed

I have an issue where the z-axis zero fails and causes a dent in my bed.  I start a print job and it goes to clean the nozzle.  After it finishes the print head moves up making a loud noise.  Then it will go to probe the bed and it drives the nozzle into the bed denting it.  It happens more than half the time I start a print job.  I've learned to stop it if I hear the noise.  The printer is a few months old with a few hundred hours on it.  This just started yesterday.  I tried power cycling, calibrating and pushing the reset button.  In order to print something I just have to keep trying to start until it doesn't happen.  I put a youtube video up that captured the noise.   Anyone else seen this or have any ideas on what could cause this?  Here is the link https://youtube.com/shorts/iupoEklFwwk

From video I can tell is not going up as it suppose to. Kit or assembled one? Probably its easy to fix, reassemble Z axis according to instruction, then run calibrate Z and post video.

mini+ [bondtech dual drive, revo, reprinted with PA11-CF and ASA] lives in photo tent.
mk4 stock (with some parts reprinted) lives in prusa enclosure.

Carnt acsecess video could you upload here please funny noise could be stepper motor on the way out so it's not moving up and down level

This was purchased assembled.   My guess is either a stepper motor is failing or the control circuit has an issue.  It doesn't happen every time.  I'm trying to see if I can figure out a pattern to the failure.

Yeah you need to live chat with prusa it's either the mother board is bad or the motor insure they send new bed to if it's damaged

Prusa suggested loosening the Trapezoidal nuts a little bit.  The printer is working currently so when the problem reappears I'll try that.  I'll update the forum msg on my findings.  It's weird because once it successfully starts a print the problem seems to disappear until I leave the printer sitting for a while.

For anyone following this thread I have some good news.  I loosened the trapezoidal nuts about 1/4 turn  and so far the problem has not reappeared.  I haven't let the printer sit for more than 12 hours without it being used.  My next test will be to let it sit a few days and verify the problem is gone.   Thanks Prusa for the great customer support!

Sounds to me like the Z access isnt fully vertical, why would loosening the screws fix that? unless they were not perpendicular to the screw rod.  I would be interested to see if you move the z top the top, remove the screws to the trapezoidal nuts and see how loose and free moving the Z is, you could also remove the top 2 caps... i had the kit and notice even though both min rods and screw drives were in perfect alignment to mount the X axis, they were both parallelogram to fit to the top caps, i had to push them both LEFT to fit, i seem to remember the same on the Mk3, this doesn't really make any sense as its a square frame... i also didnt have any issues but seems odd, so would test that to confirm there's no friction stress in the Z axis system.

 

To reset once rebuilt, make sure the z axis is near the top then run the self test...

I thought maybe the nut was pressing too hard on the rod.  It did reappear once but that was once out of about 20 starts.  Your idea is pretty good but as long as it's working I'm afraid to fiddle with it.  If it starts again I may try it.  I bought mine assembled since I didn't want to wait for the kits.  It would have helped me understand a little better if I had put this together.
