X-axis homing failed - collission detected every other print

Had issues immediately after assembly, calibration failed, x homing. After hours of troubleshooting with Prusa, suddenly worked, could finally print, out of the blue, didn't change anything. Thought I was out of the woods. First print was successful.  But every other print, the x-axis homing fails, collision detected. I print a small part to test, flawless, works perfectly. I try to print again immediately after, same print, same settings, x-axis homing fails immediately. Shut down the printer, try again after a few hours, pattern repeats. first print works, the other can't home x and says it detects a collision.

I am baffled and so is Prusa, escalated to engineers/devs, waiting to hear back

Has anyone else had these issues?

After lots of troubleshooting with Prusa, we found out that it was the X-Idler pin, hand to take off the x-axis, re-set the pin and make sure it stays locked in by pulling tight on the belt before reassembly. no issues since!

I'm also getting this, intermittently at first and now consistently.  I also got a Y-axis fail during homing at some point, but that didn't reappear.  I would suspect a homing-switch problem, but I don't see any on the machine.

I checked the motor connection to the Buddy board and the belt tension seems okay.  I can't see the area where the set-screw would land on the pulley flat-spot, but I don't think the pulley is slipping, just watching both pulley and belt as the machine repeatedly bangs itself into the left x-axis stop.

edit:  I take it back, I can see the pulley set screws - they seem fine, too.

For me, the problem seems to have been solved by updating to alpha firmware 5.1.0.  I haven't done a print yet, but I can now auto-home just fine, with no changes to belt tensions or resetting the x-idler pin.
