Here is my printer - now in stock configuration.  It can print, but still clicking noise when printing (not 100% of the time).  The looser the idler screws the less clicking and better printing - as far as I can tell.

Thanks again for any thoughts.

Hello,

In my Prusa MK4, I also began to hear clicking during operation. The clicking was associated with changing the direction of filament movement (I held it with my fingers during printer operation). After stopping the print, I noticed that the clicking also occurs when I alternately pull and push the filament myself. Do you also experience these sounds? Is there anything that needs to be done about it? I am also attaching a video:

Did you lube the gears? 
could this be retraction as the gears reverse direction? Try a test print with 0 mm retraction see if the noise goes away

An hour ago, I unscrewed the cover of the extruder gearbox to see what was going on. After unscrewing it, the knocking sound disappeared. I added a bit more grease (because previously, according to the assembly instructions, I didn't apply much), screwed the cover back on, and since then, there has been no knocking.

What Joan says is a good starting point. I would recommend that you remove the gearbox once, clean it thoroughly, reinsert it, very importantly!!! grease it properly (as in the MK4 assembly instructions) and then realign the gearbox using the MK4 menu. Above all, you should not overtighten the PG Case screws.

The clicking sound is simply the skipping of any Stepper. Mostly the Extruderstepper, sometimes the X or Y-Stepper. 

clicking from the extruder you can verify via Control/ Move Axes E. It can be also caused by low temperature or stucked filament. This can be justified by a dent on the filament after filament unload.

Prusa sent me some replacement Idler parts.  I completely disassembled the Nextruder to replace the parts.  I had properly greased it - and had followed all assembly directions (watching a couple of different assembly videos during assembly and reading Prusa manual notes at each step).

When replacing the gearbox - this time the gears turned smoothly.  Before, the gears would turn with a slight rumble.  Upon reassembly, it appears that clicking is gone.  I do have some print quality issues that I will try to fix now.  The first layer is great, but second layer and subsequent layers - gaps appear in print.

I tried what you said about the spring screws. They were not flushed when I mounted and now are even less flushed with a half turn less, and the ticking click is gone in Normal and Input shaper mode.

The only thing I hear now is the extruder moving, but I still hear things shaking inside. I will recalibrate the gears.
