Print head bed crash and broken Nextruder adapter

My Nextruder e3d v6 adapter broken into two pieces.

I went to reprint a 3d model in PETG material.

The printer did it's bed leveling routine as usual.

For some reason, bed leveling was not right and the nozzle gouged into the textured bed sheet.

After that, I tried to clean up the bed sheet but not much could be done.

The next print attempt auto bed-leveled.  The Nextruder adapter had broken into two pieces.

What was the cause?  There was a jam a few days ago that I cleared using a torch flame.  

The Nextruder adapter is not under warranty (DIY kit printers).  I ordered two OEM from "Printed Solid" (Prusa's distributor for USA) for $25 each.  Aliexpress sells clones for $7 (quality anyone?).

What could be the solution?  Detect when the print head crashes and digs into the bed.  Any case where the printer can break by itself in normal usage should be prevented.

To me, the most likely cause of this is that the 2 thumbscrews that hold the nextruder nozzle in place were too loose - during or after levelling, it slipped down, and when it started printing the tip of the nozzle was actually below the bed position and scraped across the bed.  Any chance this is what happened?

I don't think so.  The screws were tight.  If they were loose, the nozzle would have been pushed upwards by the bed and not scraping downwards.
