Are you on the latest version of firmware?
Please save your project as a .3mf file
Files > Save Project as
Zip the .3mf and post it here. It will contain both your part and your settings for us to diagnose.

Thanks folks for your help.
I'm on firmware 5.1.2 - I haven't updated to 5.1.3 because it sounds like it just adds a printer type check at startup.

Well Prusa support seem like they've ghosted me (asked me to print a temp tower, which looks fine).
I printed the exact same part in PLA, same issues but not as severe.

I am noticing some slight artifacts in the Z-axis as well, I wonder if it could be related? I'll try and get a photo.

Thanks mate, that's very interesting. TBH I was kinda suspecting a mechanical issue but if it's happening on your printer as well then it must be something else.
I tried tweaking the input shaper frequencies today, very slight improvement in ringing/ghosting but no change to the problem I'm having in this thread.
I tried drying the filament as well, no change.
