Serious issues..
Yes the plate looks nasty.. I've cleaned it a thousand times. Ive taken apart the hot end assembly several times and cleaned the crap out of it. calibrated till the cows came home and now this.... now the plate is toast. This thing is getting closer to the window it sits close by. I am at a loss

Tha'ts caused by having the extruder too close to the bed. You have damaged the pei surface with the hot nozzle. As its so close plastic wont come out properly as the gap is too small, which will be causing all sorts of back pressure in the extruder too. Adjust your live z to be further away. Back it way off and then adjust closer a bit at a time using the instructions from the Jeff Jordans Life Adjust thread.
