Idler Tension/Extruder Problems

I purchased my Prusa Mini a year or two ago and am running into an issue with the extruder. The first layer's surface has an in-and-out consistency and then the second layer usually gets worse. It's predictable and I think it is under extruding, gripping the filament again and then losing it, then repeating. I have spent a few hours trying different tensions on the idler and I tried cleaning the inside of the extruder including the teeth that grip the filament. I'm still getting the first layers with holes in them and if it gets very far into the print it grinds the filament completely and then stops producing plastic from the hot end.
Any suggestions are welcome,

I would start by taking the extruder off the printer and taking it apart enough to see if the gears are not full of filament dust.
Also check the filament path is clear and doesn't have a lot of resistance.

Thanks for the response. I cleaned out some black filament dust from the extruder but that didn't seem to fix the problem. I tried a different filament and it doesn't show the issue as badly but still has it.
Any further ideas?

Ahoj,
mám dost podobný problém.
Filament PLA teplota na trysce 215 stupňů. Hnací kolečko vyměněno za nové. Štelovaci šroub seřízen tak, že struna při čištění jde plynule neseka se. Vyzkoušeno na třech strunách. Po vyjmutí struny nejsou na struně viditelné otlačeniny ani po hmatu není nic cítit. Ale asi po hodině tisku se struna sekne (prodře) viz foto. Vytiskne se tak jedná dvě vrstvy.
