Any idea what may be wrong? I am using brand new Prusa Filament. I made no changes to the configuration.

from the picture it looks like you have had a layer shift, possible caused by the nozzle snagging on raised filament from the layer below, 
the layer below may be the first layer, it's difficult to see from the picture. 
did you get a crash detection message? 

there is very little information to go on, from the posts so far. so as a guess... it is likely that your first layer has lifted from the build plate a little, causing roughness and possibly a collision. which can lead to layer shift (note the displaces perimeters on the straight edge and around the hole. 

I would start off, by removing the print, then taking the build plate to the kitchen sink using a little dish soap, and hot water with a clean new kitchen sponge
rub the dishsoap solution all over the build plate surface to remove any finger grease and any other contaminants, then rinse well with HOT water, and dry immediately with Plain Paper towels, keeping your fingers off the build plate surface, 
then I would replace the clean dry build plate on the printer, and I would try another print.
