XL 2T Extruder skipping periodically

I have had problems with underextrusion and filament skipping on my 2H XL for a while now. It only happens on T1 and Im starting to go mad about it.

The extrusion skipping seems to be periodical, meaning that it happens once every whole turn of the extruder gear. Ive tried this on large prints and timed the underextrusion so in example on a large first layer calibration the skips happened approx every 12s. Im starting to wonder if the actual gearbox or the extruder gear is blown.

Bought it used, it is from April 2024, seemed like no issues early on at least, didnt really use toolhead 1 as much cause I was mostly doing single filament prints with 0,4 nozzle at the time. No warranty.

What I have tried and checked so far (in no order):

  • Changed the idler lever and and swivel in Prusament PCCF
  • Tried various idler tensions
  • Cleaned the extruder gear
  • Cleaned and relubed the planetary gear
  • Changed filament mid print when the underextrusion happens to check if the filament is the problem, its not.
  • forced (mild pressure feed) filament by hand during extrusion seemed like minor help- changed the print speed mid print, but seems to make no difference, except that the periodic skip changes
  • Changed the nozzle (sic 0.4) and was careful while tensioning the new nozzle, so its not kinked. This works perfect on the other toolhead

I can hear it click when underextrusion happens, also, I can even hear and observe it while purging filament.

Could the whole extruder gear or gearbox be off?

Ive been in contact with prusa support about the matter. They asked me to swap the extruder stepper of the toolheads and keep everything else the same. 

It moved the problem from T1 to T2, indicating that it is the actual extruder stepper that is faulty.
