Probing fail / Bed leveling failed

Every few prints, I get a "probing fail" / "bed leveling fail" message - during probing it seems to "cluster" probe in a specific area (back-right).

I've had success if I reset & recalibrate.  I've tried doing the tool offset recalibration by itself - which sometimes works, sometimes doesn't... and yes, the print heads are clean and I've unloaded the filament prior to calibration.  Ideas?  Thoughts? Threads someone can point me to?

I am getting the same issue, just started.  Ive been printing for months and never happened before.  I tried both sheets and made sure nothing was underneath them but no luck.

 

Same area, back right.

I also had problems with bed leveling at the beginning.
It couldn't measure properly, tried again and again and always moved the print bed a little higher until the nozzle scratched the bed so much that a crash was detected.I wrote to support and did the following:

A. Loosen the screws of the Z-motors slightly, then move the print bed all the way down and back up enough to tighten the screws
B. Grease the Z-spindles.

At this point my problem was solved, C. would have been to check whether all nozzles are pushed all the way into the extruder so that the load cell can determine a correct value.

I've had the same issue with this and "Nozzle Cleaning Failure" all at the start of the print.  I literally haven't been able to print for months since they updated the firmware.    I FOUND A FIX THOUGH, and ya'll are going to yell at me for it but, use a different printer profile than prusa.  I used my Ender 3 profile, and it prints perfectly, every single time.  SO, it's the firmware, not the printer, or you or the parts.  It is the firmware that is sh*tting the bed lol

I also had this very same problem. After a few prints refused to bed level. Back-right every time. Greasing the z-spindles immediately fixed the bed probing issue and everything appears fine for now.

I am encountering this issue and I am curious if the modular bed could lead to this? For example if one or two panels are not tightened the same amount could that lead to inconsistencies that cause the bed leveling to fail. Anecdotally as I've been troubleshooting bed leveling on my printer before I found this thread I tried adjusting the modular bed bolts, the ones that hold the modular panels, and I noticed the cluster probing moved to a different area or probed less. I think something like this is plausible but would need more testing to validate. My printer appears to have flexible washers and they are not that tight leading me to believe there could be quite a bit of differentiation between the different panels.

They don't appear to be torqued to any specification and the Prusa documentation has nothing on how tight they should be. 

Going to try the recommendation from @extrutim and will report back if that changes things for me.

Anyone have any thoughts on this?

Maybe I'm just chasing my tail.
