Large X and Y-axis offset mid-print leading to crash

I've got a print-bed sized box-shaped print that I'm doing (I've done about 10 of these before without any issue), and suddenly about half way through the second layer, the tool head does a one-inch offset on the X and Y axes and starts printing from there which leads to the print crashing because the nextruder head literally crashes into the printer frame.  I've tried this print 4 times, using several different nextruder heads on the printer, different filament spools, and I have done a belt tightening test and recalibration of x and y axes - still the same print issue at the same point of the print.  I've also updated the firmware to the latest version.

Another different thing this print is doing compared to previous prints is this weird "striping" of sets of filament passes.  The first layer always goes down very smooth, but this second layer gets these rougher sets of strips.  Anyone come across either of these issues?
